Sunset Design created a complex, interlocking system of web-based modules to accommodate the deployment and management of the FacTS survey. The modules include web-based survey instruments for both the workforce and client surveys. Survey participants complete their surveys by logging into the survey instrument with a unique password and submitting their responses. A backend web-based interface was created for survey coordinators to enter survey participant information and manage participant lists. A help desk module was created to track help desk questions and responses to ensure that participant questions and issues were resolved promptly. All data submitted through the survey instruments and participant modules is stored in a database and used to generate survey data analysis reports. A website was created to house the data analysis reports providing access to only those authorized to view the data.
